Please complete the referral form below to enroll in the Active Parenting 0–5 course. This four-week, evidence based program is designed for parents and caregivers of children from birth to age five who want to strengthen connections, build positive routines, and support healthy early development.

The course focuses on understanding child behavior, preventing challenges before they arise, encouraging positive behavior, and preparing children for school success. Sessions include guided discussion, hands on activities, and video examples to support learning in a practical and supportive environment.

Once the referral form is submitted, our team will review the information and contact the participant to discuss scheduling, enrollment, and payment details unless otherwise noted. If you have questions or need assistance completing the form, please contact our office for support.
